This private service is not presented as a replacement for facility-funded programs. We support residents who need focused one-on-one attention for mobility and daily function.

What a visit can look like

We begin by gathering context from the resident, family, and facility staff where appropriate.

The assessment focuses on mobility within the facility, safe transfers, and use of mobility aids.

Initial treatment and education are provided where appropriate, followed by a plan that balances independence with safety.

With the resident’s consent or appropriate authorization, follow-up progress may be communicated to family or facility staff while the plan is adjusted as needs change.

Facility coordination and consent

Providing care in a residential setting requires clear communication. We coordinate with caregivers and facility staff to ensure care aligns with the resident's overall plan.

Please note that all family and facility updates occur only with appropriate consent and authorization. Visits depend on facility access permission and our operational scheduling in the area.
